Class Registration Form
You'll select a class and tell us a little about you and your dog.
Payment
You will be able to secure you and your dog's place in class by paying with a credit card or PayPal.
Class Policies
The form includes Club policies about safety and security that you will need to accept to participate in class. You can preview them here.
Vaccination Records
You will also have the opportunity to upload a copy of your dog's vaccination records. For all our dogs' safety, all vaccinations must be current.
Puppies require DHPP (distemper-parvo) vaccinations.
Dogs over 6 months require rabies and DHPP.
